The Chronic Disease Prevention Initiative (CDPI) is designed to assist communities improve physical activity participation rates, address unhealthy eating, and reduce or prevent smoking. Collectively, lack of physical activity, poor nutrition and smoking are the three major risk factors that lead to chronic disease.
The next intake deadline for community grant applications that will address chronic disease prevention is Tuesday November 2.
IMPORTANT INFORMATION REGARDING CDPI IN TAKE REQUIREMENTS
The attached Application Form must be completed.
All applications must include Budget/Information details as noted in the Application Form.
Groups who fail to submit the application form and budget information section will not be given consideration.
Eligible budget line items cannot include Administrative Costs, rental of facilities, or utilities.
Core programming for costs related to snacks at schools and daycares will not be considered.
A portion of costs for Honorarium for Contract Staff is eligible (i.e. Yoga Instructors or Recreation Program Leaders).
Priority will be given to applications that are multi-week programming in nature. Low Priority consideration will be given to one-day events.
We encourage applications to be submitted as quickly as possible in the event we need to correspond with you for further clarification.
Applicants who received funding at this past July 2010 in take (and previous in takes in 2010), and who have not yet completed their projects and submitted the required activity and financial reports are not eligible for consideration for this in take.
The number of applications we receive for each in take has increased significantly. As a result, the dollar amounts we are able to allocate to groups may not be for the entire amount requested given the limited amount we have for each in take.
